Overview
STReTCH is a community- and family-based screening program in rural Telangana that integrates molecular diagnostics to identify and manage hypertension, diabetes, tuberculosis, and cervical cancer. The project leverages electronic data capture and geo-coded demographic information to ensure accurate documentation and targeted interventions.
Aim
To strengthen early detection, timely management, and referral for chronic and infectious diseases at the family level, improving health outcomes in underserved communities.
Objectives
- Update the existing geo-coded database and demographic and health information for 5,000 individuals.
- Implement a community-based, family-level screening intervention for hypertension, diabetes, and tuberculosis, cervical cancer.
- Electronically document biological parameters at the family level, including height, weight, blood pressure, serum creatinine, fasting blood glucose, and glycosylated hemoglobin (HbA1c), as required.
- Conduct molecular diagnostic testing for tuberculosis among high-risk individuals and collect self-swabs to test for human papillomavirus (HPV) DNA virus, indicative of cervical cancer.
- Facilitate management, referral, and linkage for patients, including colposcopy for cervical cancer and treatment for tuberculosis.
Project Implementation
Site:
Medchal Mandal, Telangana. Five field teams (data collector + phlebotomist) conduct household visits using electronic data capture with customized software and tablets; supervised by two Project Managers and one Coordinator; medical advice and referrals provided by a designated Medical Officer. HPV-positive participants receive counseling and are referred to hospital for further assessment and colposcopy as needed. Participants with diabetes or hypertension are provided medical and dietary advice, with newly diagnosed cases linked to appropriate care and ongoing follow-up coordinated through ASHA workers and government facilities.
