Overview

REACH 2.0 is a three-year strategic plan by SHARE INDIA to expand comprehensive non-communicable disease screening across 24 villages in Medchal through a structured, sequential rollout. The program will cover 8 villages per year over 36 months, with each village undergoing a six-week standardized screening cycle. This includes two weeks of pre-screening preparation with community mobilization and household enumeration, three weeks of active door-to-door screening with blood pressure measurement and venous blood collection for HbA1c, fasting glucose, serum creatinine, and thyroid profile, followed by one week of post-screening procedures for result communication, counseling, referral coordination, follow-up enrollment, and data reporting. Daily cold-chain transport of samples to MediCiti Hospital Laboratory will ensure laboratory quality and reliability.

To establish a scalable and replicable model for large-scale NCD screening and linkage to care in rural and peri-urban communities.

Project Implementation

All screen-positive individuals will be systematically referred to MediCiti Hospital with structured follow-up to monitor appointment adherence, treatment compliance, and outcomes. Monitoring and evaluation will track village coverage, household coverage above 95 percent, participation rates above 80 percent, disease prevalence, referral completion, and linkage to care. 

Reporting will include monthly operational updates, quarterly reviews, annual impact assessments, and a final project evaluation. With dedicated teams, optimized logistics, and strong risk mitigation strategies.

The program aims to establish a scalable and replicable model for large-scale NCD screening in rural and peri-urban settings.

Current Status

A total of 3,033 individuals were screened, and data analysis is currently in progress.
